Clinical Outcomes and Renal Safety in HIV/AIDS Patients on Tenofovir-containing Regimens in Lesotho

Abstract: Although TDF use has been associated with acceptable safety, reports of rare manifestation of renal disease in HAART regimens that include TDF have been documented. The study was conducted at Paballong HIV/AIDS care centre in Berea, Lesotho. The aim of the study was to evaluate clinical outcomes and renal safety in HIV/AIDS patients taking TDF-containing HAART regimens. Descriptive, observational, longitudinal retrospective design was followed on 255 adults on TDF-containing HAART regimens at the study area; from October 2015 to March 2016. Data captured on a data collection tool included baseline, follow-up and end-line characteristics of clinical outcomes and renal safety. Patients gained an estimated body weight of to 0.10 kg from baseline (p < 0.05) at any age. Females were on average 2.49 kg heavier than males (p < 0.05). The CD4 cell count results estimated a daily increase of 0.20 cells/mm3 at any age. The mean CD4 cell count of female patients was 69.13 cells/mm3 higher than for males (p = 0.02). The eGFR results contended that sex, age and body weight are risk factors to developing renal insufficiency. The eGFR declined by 0.78 ml/min/1.73m2 over the treatment duration at any age of treatment initiation (p < 0.05), while the average eGFR for females was lower (13.05 ml/min) (p < 0.05). Clinical outcomes manifesting by weight gain and CD4 cell count elevation improve at any age and better in females. The renal function is progressively deteriorated at any age and worsened in females.

Abstract: Objective We aim to explore the clinical effect of manual lymph drainage as a complex decongestive therapy on improving upper limb lymphedema after breast cancer operation. Methods Manual lymph drainage was performed on 28 patients with lymphedema after receiving breast cancer operation. Circumference measurement was done at transverse palmar crease, transverse carpal crease, 10cm below transverse cubital crease, 10 cm above transverse cubital crease and 20 cm above transverse cubital crease at the first day before treatment and 1 day, 1 week, 2 weeks and 3 weeks after treatment. The movement of shoulder joints, pain and numbness of patients were recorded. Results After 3 weeks of manual lymph drainage, the circumference at transverse palmar crease, transverse carpal crease, 10cm below transverse cubital crease, 10 cm above transverse cubital crease and 20 cm above transverse cubital crease was significantly lower than that before treatment. The detumescence of the middle segment of forearm (10 cm below transverse cubital crease) was better than that of the middle segment of upper arm (10 cm above transverse cubital crease) and the superior segment of upper arm (20 cm above transverse cubital crease) with a significant difference (P<0.05). After the treatment of manual lymph drainage, the life quality of patients improved greatly and there was a significant difference (P<0.05). Conclusion Manual lymph drainage as a complex decongestive therapy for lymphedema after breast cancer operation is safe, effective and thus well worth clinical application.

Abstract: The Christian family is considered as the first and basic expression of man’s social nature. The family is also the smallest social unit and a community of persons, where love and affection are demonstrated in a genuine way. In this community of persons, two partners stay together and accept each other. They give themselves in love as they profess their faith during the sacramental rite of marriage. Thus, because of the significance and the vital role the family plays in the entire community and in the society, there is the utmost need to safeguard the interest of the family, especially when religion and health issues are involved. If the family is healthy, its members will be able to worship God in truth and in serenity of hearts. The society too will remain cohesive. Hence, it is important to understand the role of cultural beliefs and how they affect the individuals in the family in order to promote the stable and functional families that make up society. Therefore, this paper aims to discuss the influence of traditional healing practices Ifa (divination), rituals and sacrifices on holistic health among the Christian families in South-West Region of Nigeria. The goal is to discover different ways to help each family member gain the right knowledge about cultural beliefs and practices in order to achieve holistic health.

Abstract: Nausea and vomiting are common experiences in pregnancy and it is considered as a part of normal physiology. It affects about 75% of pregnant women without a known cause. In most cases, it is a mild and self-limited condition that can be controlled with conservative measures and has no adverse fetal outcomes. Women with severe nausea and vomiting during pregnancy (NVP) may have hyperemesis gravidarum (HG); an entity distinct from NVP, which if left untreated may lead to significant maternal and fetal morbidity. The objective of this study was to assess the presence and severity of anxiety in pregnant women with and without HG. The study is a case-control study on 52 pregnant women divided into two groups: cases hospitalized with the diagnosis of HG and healthy pregnant women as a control group. All mothers in the study were evaluated by Hamilton Anxiety Rating Scale (HAM-A) and General Health Questionnaire (GHQ-28). It was revealed that most of hyperemesis gravidarum cases suffered from severe anxiety while only (7.7%) of healthy control had severe anxiety, with high statistical significant difference. The mean of Hamilton Anxiety Rating Scale (HAM-A) is statistically higher among hyperemesis gravidarum cases than healthy controls. Regarding General Health Questionnaire - 28 subscales; the mean score of somatic symptoms, social dysfunction and severe depression is statistically higher among hyperemesis gravidarum cases than healthy controls. It was concluded that anxiety and emotional distress were more common and severe in patients with HG.

Abstract: We recruited 15,417 participants aged 18 to 36 months in central China using a stratified, multi-stage random sampling method. For children with a positive result in Toddlers screening test, further diagnostic assessment of autism spectrum disorders (ASD) was performed using the childhood autism rating scale and the Diagnostic and Statistical Manual of Mental Disorders. ASD prevalence among these children was 0.58% in Hubei province. Multivariate-adjusted logistic regression models were then used to explore potential ASD risk factors. One parent aged 35 or older, preterm birth, and birth weight < 2.0 kg remained significantly associated with elevated ASD risk (P < 0.05). These findings indicated that management of adverse perinatal factors may prevent the occurrence of ASD.
